Who should make decisions about your end-of-life care?

Making decisions about what level of end-of-life care you wish to receive is one thing, but when there is no plan in place, who should have the authority to make decisions on your behalf?

Should the health-care professionals be the ones that have the final say as they have the experience and training to make such decisions objectively?

Or should it be your friends, family members or partners, who may know what you would want, even if it conflicts with what the medical staff are suggesting?
